Purpose

Symbesi Care and Support Coordination is a value-based enabling service dedicated to supporting adults to maintain and develop their independence in the community. We work with people from the age of 18 to the end of life, including adults with physical disabilities, learning disabilities, learning difficulties, autism, ADHD, mental health conditions, acute illness, acquired brain injuries, and older people with age-related conditions. Our purpose is to support individuals to build confidence, develop practical skills, and create a supportive infrastructure that enables them to prosper, fulfil their potential, and pursue their dreams.

Based in the West Midlands, Symbesi works collaboratively with individuals, their personal support networks, health and social care professionals, and partner agencies across the public, private, and voluntary sectors. We construct thorough, robust, and person-centred care and support plans that are goal-oriented and grounded in the SMART framework — specific, measurable, achievable, realistic, and time-bound. Our approach is designed to have minimal involvement but maximal impact, ensuring individuals develop the self-belief and capability to thrive independently.

Symbesi Care and Support Coordination will work alongside clients and their personal and professional support networks to pursue meaningful outcomes and establish the foundations for individuals to excel.   • Symbesi will support individuals to build confidence and capability in daily routines, decision‑making, and practical life skills that promote greater independence.

  • Symbesi will support individuals to engage in meaningful participation in social, recreational, and cultural activities by providing support to navigate and engage confidently with their local area, including building knowledge and confidence in using community‑wide public transport links.

  • Symbesi will assist individuals to explore, prepare for, and participate in education, training, or work opportunities that align with their goals and strengths.

  • Symbesi will support individuals to understand, organise, and complete financial tasks and essential documentation, including form-filling, to promote stability and informed decision‑making.

  • Symbesi will work alongside individuals to create a grocery list with an appropriate budget, shop for the required items, plan meals, and prepare them. Preferences, cultural needs, and dietary requirements will be considered.

  • Symbesi will support individuals to maintain a safe, organised, and habitable living environment through support with cleaning, routines, and decluttering, ensuring their home is a space they can feel proud of.

  • Symbesi will support individuals to schedule, prepare for, and attend appointments while helping the person understand information and progress the next steps.

  • Symbesi will provide an extension to an individual’s voice to ensure their needs, views, wishes, and feelings are clearly communicated and understood by professionals and services, enhancing confidence in a client’s decision‑making.

  • Symbesi will support individuals to develop and sustain meaningful relationships by encouraging social participation, communication, and connection with others.

Sunny Singh is the Company Director of Symbesi Care and Support Coordination. He is a registered social worker. He is based in the West Midlands and passionate about delivering a meaningful value-based service to the community.

Symbesi Care and Support Coordination was established by Sunny Singh, a registered social worker with experience across adult social care, mental health services, and children in care and care leavers. Sunny’s professional journey, combined with extensive voluntary work in the UK and internationally, has shaped the vision for Symbesi. His experiences in community-based projects, including time spent in Nicaragua and Uganda, have reinforced the importance of collectivism, altruism, and humility—principles that are embedded in the Symbesi ethos.
